EXAMS - AVOID BUNDLING
When an exam is given on the same day as an adjustment , distinguish the Exam as a "separate and distinct service" by appending MODIFIER -25 to your appropriate E&M billing code. Example: 99203-25
X-RAY REPORTS
For X-Rays taken outside your office: If an X-Ray report is deemed insufficient from a chiropractic standpoint, it is acceptable for you to RE-WRITE the report and bill for it by appending MODIFIERS -26 and -77 to the appropriate X-Ray code.
Example: 72100-26-77
MODIFIER -59
Modifier -59 has been replaced by four more specific modifiers:
XE - Separate Encounter
XS - Separate Structure
XP - Separate Practitioner
XU - Unusual Non-overlapping Service
Example: 97140-XS
REPORT OF FINDINGS
Since there is no official "Report of Findings" billing code, you may consider using the "Counseling Time
Override" to bill for report of findings visits.
